鹽酸右美托咪定聯(lián)合瑞芬太尼在骨科手術(shù)中的麻醉中的應(yīng)用分析

[摘要] 目的 探討分析鹽酸右美托咪定聯(lián)合瑞芬太尼在骨科手術(shù)中的麻醉效果及不良反應(yīng)。方法 從該院2018年3月—2019年2月收治的骨科手術(shù)患者中方便選取92例作為該次研究對(duì)象，分為兩組，其中2018年3月—2018年8月46例為對(duì)照組，給予瑞芬太尼聯(lián)合丙泊酚進(jìn)行麻醉;2018年9月—2019年2月46例為觀察組，給予鹽酸右美托咪定聯(lián)合瑞芬太尼進(jìn)行麻醉。比較兩組患者的麻醉效果和不良反應(yīng)。結(jié)果 兩組的麻醉效果相比，對(duì)照組麻醉時(shí)間（6.75±1.70）min，觀察組麻醉時(shí)間（5.29±1.08）分，差異有統(tǒng)計(jì)學(xué)意義（t=5.783，P=0.019）;對(duì)照組拔管時(shí)間（9.78±2.30）min，觀察組拔管時(shí)間（7.33±2.03）min，差異有統(tǒng)計(jì)學(xué)意義（t=6.437，P=0.023）;對(duì)照組VAS評(píng)分（2.41±0.23）分，觀察組VAS評(píng)分（1.53±0.42）min，差異有統(tǒng)計(jì)學(xué)意義（t=12.918，P=0.036）。兩組的不良反應(yīng)情況相比，對(duì)照組發(fā)生低血壓2例、躁動(dòng)3例、呼吸抑制3例、呼吸道梗阻3例，不良反應(yīng)發(fā)生率為23.91%（11/46）;觀察組發(fā)生低血壓1例、躁動(dòng)2例、呼吸抑制1例、呼吸道梗阻1例，不良反應(yīng)發(fā)生率為10.86%（5/46），差異有統(tǒng)計(jì)學(xué)意義（χ2=5.878，P=0.002）。 結(jié)論 鹽酸右美托咪定聯(lián)合瑞芬太尼應(yīng)用于骨科手術(shù)中麻醉效果好，可顯著降低患者不良反應(yīng)發(fā)生率，安全系數(shù)更高，值得臨床推廣應(yīng)用。

[Abstract] Objective To investigate and analyze the anesthetic effect and adverse reactions of dexmedetomidine hydrochloride combined with remifentanil in orthopedic surgery. Methods From the orthopedic surgery patients admitted in the hospital from March 2018 to February 2019， 92 cases were convenient selected as the research object and divided into two groups， of which 46 cases were from March 2018 to August 2018 as the control group. Anesthesia was given to remifentanil combined with propofol; 46 cases from September 2018 to February 2019 were the observation group， and dexmedetomidine hydrochloride combined with remifentanil was anesthetized. The anesthetic effects and adverse reactions of the two groups of patients were compared. Results Compared with the anesthesia effect of the two groups， the anesthesia time of the control group was （6.75±1.70） minutes， and the anesthesia time of the observation group was （5.29±1.08） minutes. Tube time （9.78±2.30） minutes， observation group extubation time （7.33±2.03） minutes， the difference was statistically significant（t=6.437，P=0.023）; control group VAS score （2.41±0.23） points， observation group VAS score was （1.53±0.42）points， and the difference was statistically significant（t=12.918，P=0.036）. Compared with the adverse reactions in the two groups， 2 cases of hypotension， 3 cases of agitation， 3 cases of respiratory depression， and 3 cases of respiratory obstruction occurred in the control group. The incidence of adverse reactions was 23.91% （11/46）， 2 cases of agitation， 1 case of respiratory depression， 1 case of respiratory tract obstruction， the incidence of adverse reactions was 10.86% （5/46）， and the difference was statistically significant（χ2=5.878，P=0.002）. Conclusion Dexmedetomidine hydrochloride combined with remifentanil has a good anesthetic effect in orthopedic surgery， it can significantly reduce the incidence of adverse reactions in patients， and has a higher safety factor， which is worthy of clinical application.

該次研究中兩組的不良反應(yīng)情況相比，對(duì)照組發(fā)生低血壓2例、躁動(dòng)3例、呼吸抑制3例、呼吸道梗阻3例，不良反應(yīng)發(fā)生率為23.91%（11/46）;觀察組發(fā)生低血壓1例、躁動(dòng)2例、呼吸抑制1例、呼吸道梗阻1例，不良反應(yīng)發(fā)生率為10.86%（5/46），差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。該次研究結(jié)果證實(shí)，在不良反應(yīng)發(fā)生率方面觀察組均優(yōu)于對(duì)照組，這與胡明秀等[8]的研究中，選取選擇東莞市麻涌人民醫(yī)院收治的120例骨科手術(shù)患者，隨機(jī)分為為觀察組和對(duì)照組，各60例，結(jié)果顯示觀察組實(shí)施鹽酸右美托咪定聯(lián)合瑞芬太尼麻醉不良反應(yīng)發(fā)生率11.7%優(yōu)于對(duì)照組實(shí)施瑞芬太尼聯(lián)合丙泊酚麻醉不良反應(yīng)發(fā)生率25.0%，這與該次研究結(jié)果基本一致。

綜上所述，鹽酸右美托咪定聯(lián)合瑞芬太尼應(yīng)用于骨科手術(shù)中麻醉效果顯著，安全系數(shù)高。
