前列腺癌根治手術(shù)后尿失禁患者施予優(yōu)質(zhì)護(hù)理干預(yù)的價(jià)值及效果觀察

【摘要】 目的：對(duì)前列腺癌根治手術(shù)后尿失禁患者施予優(yōu)質(zhì)護(hù)理干預(yù)的價(jià)值及效果進(jìn)行觀察評(píng)價(jià)。方法：選取2015年6月-2018年5月于筆者所在醫(yī)院接受根治術(shù)治療的前列腺癌患者56例為研究對(duì)象，以隨機(jī)數(shù)字表法將其分為優(yōu)質(zhì)化施護(hù)組與常規(guī)化施護(hù)組，每組28例，常規(guī)化施護(hù)組以前列腺癌常規(guī)圍術(shù)期護(hù)理方案施護(hù)，術(shù)后尿失禁防控主要為于術(shù)后尿管拔除后行肛提肌訓(xùn)練指導(dǎo)，優(yōu)質(zhì)化施護(hù)組施以針對(duì)術(shù)后尿失禁防控的優(yōu)質(zhì)化護(hù)理管理，比較兩組患者術(shù)后尿失禁發(fā)生率、尿失禁持續(xù)時(shí)間、尿失禁專(zhuān)項(xiàng)防控護(hù)理認(rèn)可度。結(jié)果：優(yōu)質(zhì)化施護(hù)組干預(yù)后的尿失禁發(fā)生率與尿失禁持續(xù)時(shí)間均顯著優(yōu)于常規(guī)化施護(hù)組，尿失禁專(zhuān)項(xiàng)防控護(hù)理的認(rèn)可度評(píng)分顯著較常規(guī)化施護(hù)組高，差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：將優(yōu)質(zhì)化施護(hù)模式應(yīng)用實(shí)踐于前列腺癌根治術(shù)患者圍術(shù)期護(hù)理中，可實(shí)現(xiàn)可靠度較高的尿失禁防控管理效果，深受患者高度認(rèn)可。

【Abstract】 Objective：To observe and evaluate the value and effect of quality nursing intervention for urinary incontinence patients after radical prostate cancer surgery.Method：A total of 56 patients with prostate cancer who received radical prostatectomy in our hospital from June 2015 to May 2018 were selected as research objects.By random number table method，they were divided into the high-quality nursing group and the routine nursing group，with 28 cases in each group.The routine nursing group used the routine perioperative nursing scheme for prostate cancer.The prevention and control of urinary incontinence after operation was mainly guided by levator ani muscle training after the removal of urinary catheter.The high-quality nursing group was given high-quality nursing management for prevention and control of urinary incontinence after operation.The incidence of urinary incontinence，duration of urinary incontinence，recognition of special prevention and control nursing for urinary incontinence were compared between the two groups.Result：The incidence of urinary incontinence and duration of urinary incontinence in the high-quality nursing group were significantly better than those in the routine nursing group，and the recognition score of special prevention and control nursing for urinary incontinence was significantly higher than that in the routine nursing group，the differences were statistically significant（P<0.05）.Conclusion：The application of high quality nursing model in perioperative care of patients with radical prostatectomy can achieve a high reliability of urinary incontinence prevention and control management effect，which is highly recognized by patients.

前列腺癌是老年男性常見(jiàn)的泌尿生殖系統(tǒng)腫瘤類(lèi)型，且處于發(fā)病率逐年走高狀態(tài)，前列腺癌根治術(shù)為該類(lèi)腫瘤的主流化首選疾控技術(shù)，但接受根治術(shù)后有高達(dá)60%的受術(shù)者會(huì)受尿失禁問(wèn)題困擾，在生理心理社會(huì)等多個(gè)層次與角度對(duì)患者形成痛苦折磨，故而對(duì)前列腺癌根治術(shù)者施以術(shù)后尿失禁積極防控管理至關(guān)重要，在術(shù)時(shí)謹(jǐn)慎操作嚴(yán)防盆底肌肉損傷并行膀胱頸部重建的同時(shí)，亦需于根治術(shù)圍術(shù)期提供高質(zhì)高效的專(zhuān)項(xiàng)尿失禁防控護(hù)理，如此方能提高前列腺癌根治術(shù)受術(shù)者的手術(shù)獲益度[1-3]。本研究嘗試將優(yōu)質(zhì)化施護(hù)模式應(yīng)用實(shí)踐于前列腺癌根治術(shù)患者圍術(shù)期護(hù)理中，得到較高的護(hù)理見(jiàn)效度，現(xiàn)報(bào)告如下。
