摘要
目的:探討Resight非接觸式廣角鏡與傳統(tǒng)角膜接觸鏡在增生性糖尿病視網(wǎng)膜病變(proliferative diabetic retinopathy,PDR)患者眼前后節(jié)聯(lián)合手術(shù)后眼表的差異性比較。
方法:回顧性分析2014-01/12在新疆自治區(qū)中醫(yī)院眼科住院收治的白內(nèi)障合并增生性糖尿病視網(wǎng)膜病變的患者96例96眼,其中采用白內(nèi)障超聲乳化摘除+人工晶狀體植入聯(lián)合23G玻璃體切割術(shù)輔助Resight非接觸式廣角鏡48例48眼設(shè)為試驗組,對照組48例48眼為采用傳統(tǒng)角膜接觸鏡,余均同對照組。兩組患者性別及年齡、病程均匹配。觀察術(shù)前、術(shù)后第1、7、30d的角膜厚度(corneal thickness,CT)、基礎(chǔ)淚液分泌量(Schirmer’s test,SⅠt)、淚膜破裂時間(breaking-up time, BUT)、角膜熒光染色(corneal fluorescein staining,CFS)。
結(jié)果:兩組患者術(shù)前各項指標均無統(tǒng)計學(xué)差異,兩個組手術(shù)前后各時間點的CT值總體比較差異有統(tǒng)計學(xué)意義(F時間=748.355,P=0.000;F組別=27.196,P=0.000),其中對照組術(shù)后各時間點角膜厚度均明顯增加,與術(shù)前相比,差異均有統(tǒng)計學(xué)意義(P<0.05);兩個組手術(shù)前后不同時間點SⅠt值總體比較差異有統(tǒng)計學(xué)意義(F時間=571.094,P=0.000),其中兩組術(shù)后1、7d S I t值與術(shù)前相比差異均有統(tǒng)計學(xué)意義(P<0.05),術(shù)后30d S I t值均逐漸恢復(fù)至術(shù)前水平;兩個組手術(shù)前后不同時間點的BUT值總體比較差異有統(tǒng)計學(xué)意義(F時間=843.122,P=0.000;F組別=24.664,P=0.000),其中對照組術(shù)后各時間點BUT值均縮短,與術(shù)前比較差異有統(tǒng)計學(xué)意義(P<0.05),兩個組術(shù)眼CFS值不同組間和不同時間點的變化差異均有統(tǒng)計學(xué)意義(F時間=312.093,P=0.000;F組別=16.232,P=0.000),其中,兩個組患者術(shù)后各時間點的CFS值均高于術(shù)前,差異均有統(tǒng)計學(xué)意義(P<0.05)。
結(jié)論:Resight非接觸式廣角鏡在PDR患者眼前后節(jié)聯(lián)合手術(shù)后對眼表影響小,術(shù)后干眼癥狀較輕。

0引言
隨著糖尿病的發(fā)病率不斷上升,糖尿病視網(wǎng)膜病變的患者越來越多[1],并且許多患者合并白內(nèi)障,目前,白內(nèi)障摘除術(shù)聯(lián)合玻璃體切割術(shù)成為有效的治療手段[2],然而,由于糖尿病患者的周圍神經(jīng)病變及眼表環(huán)境易受影響[3],傳統(tǒng)角膜接觸鏡輔助眼前后節(jié)聯(lián)合手術(shù)常常加重角膜及眼表的問題[4-5],這一現(xiàn)象逐漸引起臨床醫(yī)師的關(guān)注,近年來,非接觸式廣角鏡在眼眼前后節(jié)聯(lián)合手術(shù)中的應(yīng)用倍受青睞[6-7],本文就Resight非接觸式廣角鏡與傳統(tǒng)角膜接觸鏡在增生性糖尿病視網(wǎng)膜病變(proliferative diabetic retinopathy,PDR)患者眼前后節(jié)聯(lián)合手術(shù)后眼表的改變作一對比性研究,現(xiàn)報道如下。
1對象和方法
1.1對象收集2014-01/12在新疆維吾爾自治區(qū)中醫(yī)院眼科因白內(nèi)障合并PDR住院行白內(nèi)障超聲乳化摘除+人工晶狀體植入聯(lián)合23G玻璃體切割術(shù)患者96例96眼,其中試驗組采用Resight非接觸式廣角鏡48例48眼,對照組采用傳統(tǒng)角膜接觸鏡48例48眼,其余情況兩組均相同。全部患者術(shù)前空腹血糖控制到7.0mmol/L以下,餐后2h血糖控制到11.0mmol/L以下,血壓160/90mmHg以下,心、腎功能能耐受手術(shù);無眼部外傷史及手術(shù)史、無影響淚液的其他系統(tǒng)性疾病史、無角膜接觸鏡使用史。
1.2方法所有患者術(shù)前均采用裂隙燈及直接檢眼鏡檢查眼底,并行眼部超聲檢查,兩組患者的性別、年齡均匹配,兩組患者的病情分期均為糖尿病視網(wǎng)膜病變Ⅳ~Ⅵ期,試驗組48例48眼,其中男22例22眼,女26例26眼,年齡56~78(平均69.3±4.6)歲;對照組48例48眼,其中男23例23眼,女25例25眼,年齡57~77(平均68.4±4.1)歲。手術(shù)均由同一位醫(yī)師完成。術(shù)前3d滴左氧氟沙星滴眼液預(yù)防感染,所有患者術(shù)前30min行利多卡因+布比卡因4mL球后阻滯麻醉,術(shù)前15min均使用鹽酸奧布卡因點眼3次,每次間隔5min。所有患者均行白內(nèi)障超聲乳化摘除+人工晶狀體植入術(shù)+23G微創(chuàng)玻璃體切割術(shù),試驗組術(shù)中采用Resight非接觸式廣角鏡,對照組采用角膜緣金屬環(huán)縫線固定及使用傳統(tǒng)角膜接觸鏡,手術(shù)采用3.0mm角膜切口,前房注入黏彈劑后環(huán)形撕囊,水分離,囊袋內(nèi)超聲乳化晶狀體,吸除殘留皮質(zhì),植入折疊式人工晶狀體,10/0線角膜切口縫合1針,距角膜緣3.5mm的睫狀體平坦部做3個鞏膜穿刺口,切除中軸部及周邊部玻璃體,剝膜,氣液交換后行全視網(wǎng)膜光凝,根據(jù)術(shù)中眼底情況,決定玻璃體腔內(nèi)注入氣體或者硅油,所有患者術(shù)中均采用同一設(shè)備及器械,手術(shù)結(jié)束后常規(guī)給予地塞米松+利多卡因半球后注射,術(shù)后局部使用妥布霉素地塞米松眼液滴眼,每日4次,1wk后換用氯替潑諾混懸滴眼液每日4次,逐漸遞減至術(shù)后1min停藥;術(shù)后2wk內(nèi)使用妥布霉素地塞米松眼膏點眼,睡前一次;所有患者于術(shù)前及術(shù)后第1、7、30d,由同一醫(yī)師在同一檢查室進行數(shù)據(jù)采集。采集項目包括角膜厚度(corneal thickness,CT),基礎(chǔ)淚液分泌試驗 (Schirmer’s test,SⅠt)、淚膜破裂時間(breaking-up time,BUT)、角膜熒光染色(corneal fluorescein staining,CFS)評分。CT:記錄實際數(shù)值(測3次,取平均值);SⅠt:記錄實際讀數(shù),超過30mm者以30mm記。BUT:記錄實際時間。CFS評分采用四象限評分法[8],一象限內(nèi)無著色為0分,少量(<5)點狀著色為1分,多量(>5)點狀著色為2分,伴有片狀著色或有絲狀物為3分,四象限分值相加為最后得分。
2結(jié)果
2.1兩組術(shù)后各時間點CT值試驗組術(shù)后各時間點CT值無明顯變化,與術(shù)前相比,差異無統(tǒng)計學(xué)意義(P>0.05),但對照組術(shù)后各時間點的CT值均明顯增加,與術(shù)前相比差異有統(tǒng)計學(xué)意義(P<0.05)。兩組手術(shù)前后不同時間點CT值的總體比較差異有統(tǒng)計學(xué)意義(F時間=748.355,P=0.000;F組別=27.196,P=0.000;F交互作用=165.332,P=0.000),見表1。
2.2兩組術(shù)后SⅠt值兩組術(shù)后1、7d,SⅠt值均明顯高于術(shù)前值,差異有統(tǒng)計學(xué)意義(P<0.05),以對照組更明顯,但術(shù)后30d逐漸恢復(fù)到術(shù)前水平,差異無統(tǒng)計學(xué)意義(P>0.05)。兩組手術(shù)后不同時間點SⅠt值的總體比較差異有統(tǒng)計學(xué)意義(F時間=571.094,P=0.000),兩組術(shù)后各時間點SⅠt值相比較差異無統(tǒng)計學(xué)意義(F組別=2.596,P=0.079),見表2。

2.3兩組手術(shù)前后的BUT變化試驗組手術(shù)前后的BUT變化差異無統(tǒng)計學(xué)意義(P>0.05),但是對照組術(shù)后各時間點的BUT值與術(shù)前比較差異均有統(tǒng)計學(xué)意義(P<0.05)。兩組手術(shù)前后不同時間點的BUT值總體比較差異有統(tǒng)計學(xué)意義(F時間=843.122,P=0.000;F組別=24.664,P=0.000;F交互作用=187.334,P=0.000),見表3。
2.4兩組手術(shù)前后的CFS變化兩組術(shù)后1、7、30d與術(shù)前相比,CFS均明顯增加,差異均有統(tǒng)計學(xué)意義(P<0.05),對照組術(shù)后CFS均高于試驗組。兩組術(shù)眼CFS值不同組間和不同時間點的變化差異均有統(tǒng)計學(xué)意義(F時間=312.093,P=0.000;F組別=16.232,P=0.000;F交互作用=3.860,P=0.010),見表4。
3討論
引起干眼的原因復(fù)雜多樣,手術(shù)導(dǎo)致的淚膜不穩(wěn)定及淚液分泌量的異常以及眼表的變化逐漸引起眼科學(xué)者的注意,近年隨著糖尿病的發(fā)病率逐年上升,糖尿病視網(wǎng)膜病變合并白內(nèi)障的患者在臨床中越來越常見,有研究報道[9-10]:糖尿病患者的淚膜功能較差及角膜的敏感度較正常人低,故手術(shù)后更易致干眼的發(fā)生,這種現(xiàn)象尤其在合并有糖尿病性視網(wǎng)膜病變的患者中表現(xiàn)更著。Yorek等[11]分別對患有1型糖尿病和2型糖尿病的小鼠作了一項研究,結(jié)果顯示:2型糖尿病的小鼠更易因失去神經(jīng)營養(yǎng)作用發(fā)生角膜神經(jīng)改變及周圍神經(jīng)的病變。因此,糖尿病患者在行超聲乳化白內(nèi)障吸除術(shù)聯(lián)合玻璃體切割術(shù)后角結(jié)膜上皮細胞生長、基底膜的更新明顯改變,導(dǎo)致角結(jié)膜上皮缺損,愈合延遲,與非糖尿病患者相比更易致淚膜功能改變[12]。本組資料采用評價干眼及眼表系統(tǒng)的四個常用指標(角膜厚度、基礎(chǔ)淚液分泌量、淚膜破裂時間,角膜熒光染色)來對比分析Resight非接觸式廣角鏡與傳統(tǒng)角膜接觸鏡對角膜及眼表影響的差異。采用Resight非接觸式廣角鏡,術(shù)后角膜厚度與術(shù)前比較無統(tǒng)計學(xué)差異,術(shù)后基本不引起角膜水腫,但是對照組術(shù)后角膜厚度明顯增加,考慮與術(shù)中角膜接觸鏡的持續(xù)摩擦及手術(shù)時間較長有關(guān),對照組術(shù)中需要助手使用頂棒頂壓視網(wǎng)膜后極部及周邊部,這在一定程度上導(dǎo)致結(jié)膜水腫加重,破壞了結(jié)膜囊的杯狀細胞及淚膜的脂質(zhì)層及黏蛋白層,術(shù)后反射性淚液分泌量增加,淚膜功能破壞,淚膜的破裂時間縮短,角膜熒光染色評分較試驗組高,因此,術(shù)中減少對角膜、結(jié)膜的刺激甚至是創(chuàng)傷就顯得尤為重要。近年來,國內(nèi)的彭靈等[13]報道了Resight非接觸式廣角鏡在眼前后節(jié)聯(lián)合手術(shù)中應(yīng)用,分析了其獨特的優(yōu)勢:術(shù)中與眼球不接觸,避免了術(shù)中與角膜及眼表的摩擦,觀察角度廣,術(shù)中助手只需輕輕頂壓甚至無需頂壓周邊視網(wǎng)膜即可完成手術(shù),這在一定程度上避免破壞瞼板腺體及結(jié)膜的杯狀細胞,與縫合式金屬環(huán)固定角膜接觸鏡相比,不僅縮短了手術(shù)時間,而且減少了術(shù)中機械性創(chuàng)傷及術(shù)后縫線的刺激,進一步維持淚膜的穩(wěn)定性。本組研究也存在不足之處,未能比較兩組患者手術(shù)時間對術(shù)后干眼的影響因素,需要進一步完善。針對糖尿病患者這一特殊群體,早期診斷并及時治療干眼癥對術(shù)后維持眼表穩(wěn)定性及視力的恢復(fù)具有重要意義,對于具備手術(shù)指征的患者,我們建議:應(yīng)該在臨床上進一步推廣非接觸式廣角鏡的應(yīng)用。

Abstract
?AIM: To evaluate the difference of ocular surface between Resight non-contact wide-angle lens and conventional corneal contact lens in the patients with proliferative diabetic retinopathy(PDR) after phacoemulsification combined with intraocular lens(IOL) implantation and vitrectomy.
?METHODS: A retrospective cases-controlled study was designed.Ninety-six patients (96 eyes)with PDR and cataract were included in this study from January 2014 to December 2014 in Traditional Chinese Medical Hospital of Xinjiang Uygur Autonomous Region.The 48 cases(48 eyes)in experiment group were treated with Resight non-contact wide-angle lens,the 48 cases(48 eyes) in control group with conventional corneal contact lens. Corneal thickness(CT), Schirmer’s test(SⅠt),breaking-up time (BUT) and corneal fluorescein staining(CFS) were taken before operations and at 1d,1wk and 1mo after operations.
?RESULTS: All indicators of the two groups had no significant differences preoperatively. Significant differences were found on CT value between the experiment group and control group as well as among 4 time points(Ftime=748.355,P=0.000;Fgroup=27.196,P=0.000). The CT value of the control group increased obviously after surgeries, the differences were significant compared with preoperative (P<0.05). The SⅠt of the two groups among the 4 points were significantly different (Ftime=571.094,P=0.000). The SⅠt of the two groups at 1d and 1wk were significantly different compared with those preoperative(P<0.05). The SⅠt of the two groups at 1mo postoperatively recovered to the same level as before surgeries. The BUT value between the two groups as well as among 4 time points were significantly different (Ftime=843.122,P=0.000;Fgroup=24.664,P=0.000). The BUT decreased after surgeries and the differences were significant, compared with those before surgeries (P<0.05). The CFS value between the two groups as well as among 4 time points were significantly different(Ftime=312.093,P=0.000;Fgroup=16.232,P=0.000). The CFS value after surgeries was higher than those before surgeries and the differences were significant (P<0.05).
?CONCLUSION: Resight non-contact wide-angle lens has little influence on the ocular surface in the patients with PDR after phacoemulsification combined with IOL implantation and vitrectomy.
