金梔潔齦含漱液對錯牙合畸形矯治中牙齦炎療效及齦溝液sICAM-1、IL-1β、PGE2變化的影響

[摘要]目的：探討金梔潔齦含漱液對錯牙合畸形矯治后牙齦炎患者的臨床效果。方法：選取筆者醫(yī)院口腔門診實施錯牙合畸形矯治后出現(xiàn)牙齦炎的100例患者，根據(jù)患者就診順序分為試驗組和對照組，每組50例。兩組均予以鹽酸米諾環(huán)素+口腔衛(wèi)生維護(hù)治療，試驗組同時給予金梔潔齦含漱液每日3次含漱治療。對比治療前后患者的牙齦指數(shù)（GI）、菌斑指數(shù)（PLI）、齦溝出血指數(shù)（SBI）、探診深度（PD）、齦溝液中可溶性細(xì)胞黏附分子-1（sICAM-1）、前列腺素E2（PGE2）及白細(xì)胞介素-1β（IL-1β）的變化。結(jié)果：治療前對兩組患者的GI、PLI、ABI、PD值進(jìn)行對比，差異無統(tǒng)計學(xué)意義（PP>0.05）;治療后，試驗組患者的GI、PLI、ABI、PD值均低于對照組（P<0.05）;治療前對兩組患者的齦溝液中的sICAM-1、IL-1β、PGE2檢測值進(jìn)行對比，差異無統(tǒng)計學(xué)意義（P>0.05）;治療后，試驗組患者的齦溝液中的sICAM-1、IL-1β、PGE2檢測值均低于對照組（P<0.05）。結(jié)論：金梔潔齦含漱液治療錯牙合畸形矯治后牙齦炎患者，能有效減輕牙齦組織的炎癥反應(yīng)、改善牙周健康指標(biāo)并減輕患者的疼痛感。

Abstract： Objective? To explore the clinical effect of Jinzhijie sputum containing sputum on patients with gingivitis after malocclusion. Methods 100 patients with gingivitis after the implementation of malocclusion in the oral cavity of our hospital were divided into the experimental group and the control group according to the order of the patients. Both groups were treated with minocycline hydrochloride + oral hygiene maintenance， and the experimental group was given Jinzhijie sputum containing sputum 3 times a day. The changes of GI， PLI， SBI， PD， sICAM-1， PGE2， IL-1β were compared before and after treatment.? Results The GI， PLI， ABI and PD values of the two groups were compared before treatment， and the difference was not statistically significant （P>0.05）. After treatment， the GI， PLI， ABI and PD values of the experimental group were lower than those of the control group （P<0.05）. The detection values of sICAM-1， IL-1β and PGE2 in gingival crevicular fluid of the two groups were compared before treatment， and the difference was not statistically significant （P>0.05）. After treatment， the detection values of sICAM-1， IL-1β and PGE2 in the gingival crevicular fluid of the experimental group were lower than those of the control group （P<0.05）. Conclusion Jinzhijie sputum containing sputum in the treatment of malocclusion after malocclusion treatment can effectively reduce the inflammatory response of gingival tissue， improve periodontal health indicators， and reduce the pain of patients.

固定正畸治療是目前矯正錯牙合畸形的主要方法。固定正畸治療可以更好地控制牙齒移動，獲得良好的正畸效果[1]。然而，在固定正畸治療過程中，牙齒表面與固定正畸治療結(jié)合，這使得機(jī)械去除牙菌斑和沉積牙菌斑更加困難。另外，矯正的平均時間長達(dá)一年半至兩年甚至更長，因此，牙菌斑對牙齦形成長期刺激并導(dǎo)致牙齦炎[2]。牙齦炎是固定正畸治療中常見的并發(fā)癥，由多種炎癥因子參與的一種炎癥。主要表現(xiàn)為牙齦腫脹、出血、脹痛等癥狀，未能及時治療可能傷到牙周組織并形成牙周炎，嚴(yán)重影響患者的口腔衛(wèi)生和正常飲食[5]。有效殺滅牙周菌斑是錯牙合畸形矯治后牙齦炎患者治療的根本方法。金梔潔齦含漱液是一種藥物制劑，由純天然的中藥組成，可以清熱解毒，消腫止痛。適用于緩解牙齦、牙周及黏膜炎癥所致的腫痛[4]。本次研究選取筆者醫(yī)院口腔門診實施錯牙合畸形矯治后出現(xiàn)牙齦炎的患者100例，探討金梔潔齦含漱液對錯牙合畸形矯治后牙齦炎患者的臨床效果。
