Q開(kāi)關(guān)1?064nmNd:YAG激光聯(lián)合珍珠美白散治療黃褐斑療效探討

[摘要]目的：探討Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療黃褐斑的臨床療效。方法：選取2016年4月-2019年4月在筆者醫(yī)院接受治療的黃褐斑患者110例作為研究對(duì)象，采用隨機(jī)數(shù)字表法將其分為觀察組（n=56）和對(duì)照組（n=54）。觀察組采用Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療，對(duì)照組采用Q開(kāi)關(guān)1 064nm Nd：YAG激光治療。對(duì)比兩組臨床療效、皮損總積分、積分下降指數(shù)、不良反應(yīng)率及復(fù)發(fā)率。結(jié)果：治療后，觀察組臨床總有效率明顯高于對(duì)照組（92.86 vs 66.67）%（P<0.05）;觀察組皮損總積分明顯低于對(duì)照組（1.27±0.26 vs 2.87±0.38）分，且積分下降指數(shù)明顯高于對(duì)照組（0.66±0.07 vs 0.52±0.06）（P<0.05）;觀察組不良反應(yīng)率明顯低于對(duì)照組（3.57 vs 53.70）%（P<0.05）;觀察組復(fù)發(fā)率明顯低于對(duì)照組（1.79 vs 18.52）%（P<0.05）。結(jié)論：Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療黃褐斑臨床療效理想，有效改善皮損狀況，且對(duì)降低復(fù)發(fā)率及不良反應(yīng)率效果顯著，值得在臨床上推廣及應(yīng)用。

Abstract： Objective? To Study on the clinical efficacy of Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder in the treatment of chloasma. Methods? 110 cases of chloasma patients who received treatment in our hospital from April 2016 to April 2019 were selected as the study subjects， and were divided into observation group （n=56） and control group （n=54） by random number table method. The observation group was treated with Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder， while the control group was treated with q-switch 1 064nm Nd：YAG laser. The clinical efficacy， total score of skin lesions， index of decline in score， adverse reaction rate and recurrence rate were compared between the two groups. Results? After treatment， the total clinical effective rate of the observation group was significantly higher than that of the control group （92.86vs66.67）% （P<0.05）.The total score of skin lesions in the observation group was significantly lower than that in the control group （1.27±0.26 vs 2.87±0.38）， and the index of decline was significantly higher than that in the control group（0.66±0.07vs 0.52±0.06）（P<0.05）. The adverse reaction rate in the observation group was significantly lower than that in the control group （3.57 vs 53.70）% （P<0.05）.The recurrence rate in the observation group was significantly lower than that in the control group （1.79 vs 18.52）% （P<0.05）. Conclusion? Q switch 1 064nm Nd：YAG laser combined with pearl whitening powder has an ideal clinical effect in the treatment of chloasma， which can effectively improve the condition of skin lesions and significantly reduce the recurrence rate and adverse reaction rate. Therefore， it is worth promoting and applying in clinical practice.

美白散全方有活血化瘀，行氣健脾，利潤(rùn)美白之功[17]。珍珠美白散是筆者醫(yī)院皮膚科根據(jù)多年的臨床經(jīng)驗(yàn)研發(fā)出的一個(gè)中藥外用方。由珍珠粉、白僵蠶、白芷、白茯苓、白芨、玫瑰花及白蒺藜等藥物組成。其中珍珠粉可令皮膚柔滑白嫩、收斂生肌;白芷可散風(fēng)除濕，通竅止痛;白僵蠶可祛風(fēng)止癢，散結(jié)解毒;白蒺藜可平肝明目、疏肝郁結(jié);白茯苓可利水滲濕、健脾補(bǔ)中、寧心安神;白芨可止血消腫、善治面瘡;玫瑰花氣味芬芳，調(diào)和藥物，具有較強(qiáng)的成膜性[18-19]。以上藥方均有疏肝健脾、行氣活血、祛斑美白之功，可奏潤(rùn)膚美白，祛黑消斑之效。因此，筆者醫(yī)院采用Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療黃褐斑。珍珠美白散用以提高黃褐斑患者的臨床療效及減少?gòu)?fù)發(fā)率的發(fā)生，同時(shí)使患者皮膚更具光澤度，增添患者自信[20]。

本研究結(jié)果顯示，Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療黃褐斑患者可有效提高臨床療效，聯(lián)合治療臨床總有效率92.86%，明顯高于單一采用激光治療后臨床總有效率66.6%;從圖1可以看出，聯(lián)合治療后黃褐斑明顯消退，色澤光亮，還有美白效果，單一激光治療后的效果不明顯，且色素暗沉。同時(shí)聯(lián)合治療后的皮損總積分顯著低于單一采用激光治療后的皮損總積分，積分下降指數(shù)顯著高于單一采用激光治療;聯(lián)合治療后的不良反應(yīng)率3.57%明顯低于單一治療的不良反應(yīng)率53.70%;半年后，隨訪發(fā)現(xiàn)單一治療的復(fù)發(fā)率18.52%，明顯高于聯(lián)合治療后的復(fù)發(fā)率1.79%，兩種治療方法差異顯著，說(shuō)明聯(lián)合治療的效果更佳，是一種安全、有效的治療方法。

綜上所述，Q開(kāi)關(guān)1 064nm Nd：YAG激光聯(lián)合珍珠美白散治療黃褐斑患者，對(duì)提高臨床療效、降低復(fù)發(fā)率、預(yù)防激光治療后炎癥性色素沉著效果顯著，且不良反應(yīng)少，美白效果理想，使用方便，值得在臨床上推廣應(yīng)用。
