[摘要] 目的 探討腦電圖和經(jīng)顱多普勒對(duì)兒童偏頭痛的臨床價(jià)值。 方法 方便選取2015年8月—2016年8月來(lái)該院治療偏頭痛的患兒52例,為實(shí)驗(yàn)組,對(duì)患者發(fā)作期、發(fā)作間歇期進(jìn)行EEG(腦電圖)、TCD(多普勒超聲)檢查。并選取同時(shí)期來(lái)該院進(jìn)行健康體檢的患兒52例,為對(duì)照組。將兩組情況進(jìn)行比較分析。 結(jié)果 52例患兒中,經(jīng)TCD檢測(cè)顯示腦血流動(dòng)力學(xué)正常的有8例(15.38%),腦血流動(dòng)力學(xué)出現(xiàn)異常者44例(84.62%),其中血流速度減慢患兒有13例,血流速度增高患兒有12例,雙側(cè)血流速度不對(duì)稱有14例,血流不穩(wěn)定患兒有5例。52例偏頭痛患兒中,描記腦電圖表現(xiàn)出現(xiàn)異常的患兒有24例(46.15%),其中輕度異常10例,中度異常7例,重度異常3例,癎樣放電4例,正常范圍28例(53.75%)。在兩組檢查中,經(jīng)TCD檢查與EEG檢查共同表現(xiàn)異常的有11例。經(jīng)過(guò)TCD檢查EEG檢查的患者結(jié)果與臨床病理檢查符合率分別為94%和93%,差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。 結(jié)論 對(duì)偏頭痛患兒行腦電圖和經(jīng)顱多普勒檢查,診斷準(zhǔn)確性較高,對(duì)于后續(xù)治療、用藥也有較大的借鑒價(jià)值,臨床價(jià)值較高,值得在臨床上推廣應(yīng)用。

Research on Clinical Value of Electroencephalogram and Transcranial Doppler Sonography on the Migraine in Children

[Abstract] Objective To discuss the clinical value of electroencephalogram and transcranial Doppler sonography on the migraine in children. Methods Conclusion selection 52 cases of children with migraine treated in our hospital from August 2015 to August 2016 were selected and the patients were examined by the EEG and TCD during the onset period and gointoaction the intermittment period, at the same time, 52 cases of healthy physical examination children in our hospital at the same period were selected as the control group, and the situations were compared and analyzed between the two groups. Results Of the 52 cases of children, the TCD test showed that there were 8 cases whose cerebral hemodynamics was normal, accounting for 15.38%, 44 cases whose cerebral hemodynamics was abnormal, accounting for 84.62%, 13 cases whose blood flow velocity slowed down, 12 cases whose blood flow velocity increased and 14 cases whose bilateral blood flow rate was asymmetric, and 5 cases whose blood flow was not steady, of 52 cases of children with migraine, the electroencephalogram showed that there were 24 cases were abnormal, accounting for 46.15%, including 10 mild abnormal cases, 7 cases moderate abnormal cases and 3 severe abnormal cases, 4 cases with epileptiform discharge, and 28 cases were in the normal range, accounting for 53.75%, and the TCD examination and EEG examination showed there were 11 abnormal cases, and the coincidence rate of TCD examination and EEG examination and clinical pathology examination was respectively 94% and 93%, and the difference was not obvious, without statistical significance(P>0.05). Conclusion The accuracy of electroencephalogram and transcranial Doppler sonography on the migraine in children is higher, which is of great reference value to the subsequent treatment and medication with higher clinical value, and it is worth clinical promotion and application.
