【摘要】 目的 比較分析美托洛爾與卡維地洛對(duì)高血壓患者的血壓控制效果及糖脂代謝的影響。方法 162例高血壓患者, 采取分層隨機(jī)法分為美托洛爾組與卡維地洛組, 每組81例。美托洛爾組采取美托洛爾治療, 卡維地洛組采取卡維地洛治療。比較兩組患者的血壓控制效果及糖脂代謝水平。
結(jié)果 兩組患者治療后舒張壓、收縮壓比較差異均無統(tǒng)計(jì)學(xué)意義(P>0.05)??ňS地洛組患者空腹血糖、三酰甘油(TG)、總膽固醇(TC)、低密度脂蛋白膽固醇(LDL-C)水平分別為(5.65±0.34)、(1.78±0.21)、(5.79±1.00)、(3.49±0.59)mmol/L, 均低于美托洛爾組的(6.09±0.41)、(2.21±0.40)、(6.20±1.17)、(3.80±
0.68)mmol/L, 高密度脂蛋白膽固醇(HDL-C)水平(1.51±0.20)mmol/L高于美托洛爾組的(1.25±
0.14)mmol/L, 差異均具有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 卡維地洛在降壓的同時(shí)對(duì)患者糖脂代謝有著正面影響, 較美托洛爾更加全面, 總體療效也更有優(yōu)勢(shì), 具有較高的臨床價(jià)值。

【Abstract】 Objective To compare and analyze the effect of metoprolol and carvedilol on blood pressure control and glycolipid metabolism in hypertension patients. Methods A total of 162 hypertension patients were divided by stratified random method into metoprolol group and carvedilol group, with 81 cases in each group. Metoprolol group was treated with metoprolol, and carvedilol group was treated with carvedilol. Comparison were made on blood pressure control effect and glycolipid metabolism level between two groups. Results Both groups had no statistically significant difference in diastolic blood pressure and systolic blood pressure after treatment (P>0.05). Carvedilol group had lower fasting blood glucose, triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) respectively as (5.65±0.34), (1.78±0.21), (5.79±1.00) and (3.49±0.59) mmol/L
than (6.09±0.41), (2.21±0.40), (6.20±1.17) and (3.80±0.68) mmol/L in metoprolol group, and higher high density lipoprotein cholesterol (HDL-C) as (1.51±0.20) mmol/L than (1.25±0.14) mmol/L in metoprolol group. Their difference was statistically significant (P<0.05). Conclusion Carvedilol has a positive effect on glycolipid metabolism while lowering blood pressure in patients with hypertension, and it is more comprehensive than metoprolol, and its overall efficacy is also more advantageous. It contains high clinical value.

高血壓是常見的疾病, 并且由于飲食結(jié)構(gòu)改變, 污染等因素, 其發(fā)生率一直在不斷上升。原發(fā)性高血壓不僅自身難以治療, 還會(huì)引起許多高血壓相關(guān)并發(fā)癥, 因此成為了健康殺手[1]。經(jīng)現(xiàn)代研究發(fā)現(xiàn), 高血壓和高血脂呈相互影響的關(guān)系, 兩者可相互促成, 嚴(yán)重影響患者的健康, 因此對(duì)高血壓患者僅采取血壓控制藥物并不足以保證患者生活質(zhì)量。如美托洛爾等降壓藥物對(duì)患者糖脂代謝影響小, 對(duì)血脂水平作用不明顯, 越來越難以令醫(yī)師、患者滿意[2]。本研究對(duì)高血壓患者采用了第三代降壓藥物卡維地洛, 并且與美托洛爾對(duì)照觀察患者血壓以及糖脂代謝水平, 以確定其療效, 現(xiàn)報(bào)告endprint
