老年心力衰竭急性發(fā)作期患者血鈉、B 型鈉尿肽和尿酸水平變化及其與心功能的關(guān)系研究

目的探討老年心力衰竭急性發(fā)作期患者血鈉(Na)、B型鈉尿肽(BNP)和尿酸(UA)水平變化及其與心功能的關(guān)系。方法選取2011—2013年重慶市永榮礦業(yè)有限公司總醫(yī)院收治的老年心力衰竭急性發(fā)作期患者60例作為疾病組,選擇同期老年心力衰竭穩(wěn)定期患者60例作為疾病對(duì)照組,另選擇同期門(mén)診體檢健康者70例作為對(duì)照組。比較3組受試者血Na、BNP、UA水平,比較疾病組不同心功能分級(jí)患者血Na、BNP、UA水平變化,分析其與心功能分級(jí)的相關(guān)性。結(jié)果疾病組患者血BNP、UA水平高于疾病對(duì)照組,血Na水平低于疾病對(duì)照組(P<0.05);疾病對(duì)照組患者血BNP、UA水平高于對(duì)照組,血Na水平低于對(duì)照組(P<0.01)。疾病組患者中心功能分級(jí)Ⅳ級(jí)者血BNP、UA水平高于Ⅲ級(jí)者和Ⅱ級(jí)者,Ⅲ級(jí)者高于Ⅱ級(jí)者(P<0.05);Ⅳ級(jí)者血Na水平低于Ⅲ級(jí)者和Ⅱ級(jí)者,Ⅲ級(jí)者低于Ⅱ級(jí)者(P<0.05)。Pearson相關(guān)分析結(jié)果顯示,老年心力衰竭急性發(fā)作期患者血Na水平與心功能分級(jí)呈負(fù)相關(guān)(r=-0.602,P=0.006),血BNP水平與心功能分級(jí)呈正相關(guān)(r=0.551,P=0.012),血UA水平與心功能分級(jí)呈正相關(guān)(r=0.498,P=0.021)。結(jié)論老年心力衰竭急性發(fā)作期患者血BNP、UA水平升高,血Na水平降低,且血Na、BNP、UA水平與患者心功能分級(jí)相關(guān)。

心力衰竭為心臟器質(zhì)性病變引起的臨床綜合征,近年來(lái)隨著我國(guó)人口老齡化進(jìn)程加劇,其發(fā)病率呈上升趨勢(shì),因此盡早診治心力衰竭具有重要的臨床意義[1]。鈉(Na)在心臟活動(dòng)中具有重要作用;B型鈉尿肽(BNP)為心室合成的一種重要神經(jīng)激素;尿酸(UA)為人體代謝產(chǎn)物,在心力衰竭中呈異常表達(dá)[2]。研究顯示,血Na、BNP及UA與心血管疾病的發(fā)生發(fā)展相關(guān)[3-6]。為此,本研究探討了老年心力衰竭急性發(fā)作期患者血Na、BNP及UA水平變化及其與心功能的關(guān)系,現(xiàn)報(bào)道如下。
1.1 納入和排除標(biāo)準(zhǔn)
1.1.1 納入標(biāo)準(zhǔn)[5](1)符合心力衰竭的診斷標(biāo)準(zhǔn); (2)年齡≤85歲;(3)無(wú)惡性腫瘤、嚴(yán)重肝腎功能不全及自身免疫性疾病;(4)近3個(gè)月內(nèi)未接受抗炎、激素、免疫抑制劑治療;(5)心功能分級(jí)Ⅱ~Ⅳ級(jí);(6)依從性佳;(7)知情同意。
1.1.2 排除標(biāo)準(zhǔn)[5](1)慢性阻塞性肺疾病、支氣管哮喘、2型糖尿病、炎性疾病及代謝性疾病者;(2)急、慢性感染者;(3)嚴(yán)重營(yíng)養(yǎng)不良者;(4)有其他嚴(yán)重精神疾病者;(5)有酒精、藥物濫用史者;(6)不配合接受實(shí)驗(yàn)室檢查者;(7)未完成隨訪(fǎng)者。
1.2 一般資料選取2011—2013年重慶市永榮礦業(yè)有限公司總醫(yī)院收治的老年心力衰竭急性發(fā)作期患者60例作為疾病組,其中男32例,女28例;年齡61~85歲,平均(65±4)歲;紐約心臟病協(xié)會(huì)(NYHA)心功能分級(jí):Ⅱ級(jí)24例,Ⅲ級(jí)20例,Ⅳ級(jí)16例。選擇同期老年心力衰竭穩(wěn)定期患者60例作為疾病對(duì)照組,其中男28例,女32例;年齡62~85歲,平均(64 ±3)歲;另選擇同期門(mén)診體檢健康者70例作為對(duì)照組,其中男34例,女36例;年齡63~85歲,平均(67±6)歲。3組受試者年齡、性別間具有均衡性,均知情同意并簽署知情同意書(shū)。
1.3 研究方法
1.3.1 標(biāo)本采集于清晨空腹?fàn)顟B(tài)下抽取受試者外周靜脈血5 ml,3 500 r/min離心10 min,取上清液-80℃保存待測(cè)。
1.3.2 標(biāo)本檢測(cè)采用全自動(dòng)生化分析儀檢測(cè)受試者血Na和UA水平,采用熒光免疫法床旁檢測(cè)儀(Triage,Biosite)檢測(cè)受試者血BNP水平;試劑盒購(gòu)自上海華大科技有限公司,嚴(yán)格按照說(shuō)明書(shū)進(jìn)行操作[5]。
1.4 觀察指標(biāo)比較3組受試者血Na、BNP、UA水平,比較疾病組不同心功能分級(jí)患者血Na、BNP、UA水平及其與心功能分級(jí)的相關(guān)性。
1.5 統(tǒng)計(jì)學(xué)方法使用SPSS 17.0統(tǒng)計(jì)學(xué)軟件進(jìn)行數(shù)據(jù)處理,計(jì)量資料以(±s)表示,多組間比較采用單因素方差分析,組間兩兩比較采用SNK-q檢驗(yàn);相關(guān)性分析采用Pearson相關(guān)性分析。以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2.1 3組受試者血Na、BNP及UA水平比較3組受試者血Na、BNP及UA水平比較,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。疾病組患者血BNP、UA水平高于疾病對(duì)照組,血Na水平低于疾病對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);疾病對(duì)照組患者血BNP、UA水平高于對(duì)照組,血Na水平低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.01,見(jiàn)表1)。

2.2 疾病組不同心功能分級(jí)患者血Na、BNP及UA水平疾病組不同心功能分級(jí)患者血Na、BNP及UA水平比較,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。疾病組患者中心功能分級(jí)Ⅳ級(jí)者血BNP、UA水平高于Ⅲ級(jí)者和Ⅱ級(jí)者,Ⅲ級(jí)者高于Ⅱ級(jí)者(P<0.05);心功能分級(jí)Ⅳ級(jí)者血Na水平低于Ⅲ級(jí)者和Ⅱ級(jí)者,Ⅲ級(jí)者低于Ⅱ級(jí)者,差異有統(tǒng)計(jì)學(xué)意義(P<0.05,見(jiàn)表2)。

2.3 相關(guān)性分析Pearson相關(guān)分析結(jié)果顯示,老年心力衰竭急性發(fā)作期患者血Na水平與心功能分級(jí)呈負(fù)相關(guān)(r=-0.602,P=0.006),血BNP水平與心功能分級(jí)呈正相關(guān)(r=0.551,P=0.012),血UA水平與心
功能分級(jí)呈正相關(guān)(r=0.498,P=0.021)。
電解質(zhì)水平、神經(jīng)激素、機(jī)體代謝產(chǎn)物與心力衰竭的發(fā)生和發(fā)展密切相關(guān)[7],且在心力衰竭的進(jìn)展過(guò)程中起重要作用[8-12]。Na+為人體電解質(zhì)的主要成分之一,其在心臟活動(dòng)中具有重要作用;BNP為目前研究較多的神經(jīng)內(nèi)分泌激素,研究顯示,BNP與心力衰竭的發(fā)生發(fā)展密切相關(guān)[13];UA為機(jī)體代謝產(chǎn)物,其在心力衰竭中呈異常表達(dá)。Yoshihiko等[5]研究發(fā)現(xiàn),心力衰竭患者血清BNP水平高于健康人群。本研究結(jié)果顯示,疾病組和疾病對(duì)照組患者血Na水平低于對(duì)照組,而血BNP、UA水平高于對(duì)照組,表明血Na、BNP、UA水平與心力衰竭的發(fā)生發(fā)展有關(guān)。
Antonopoulos等[6]研究發(fā)現(xiàn),不同病情嚴(yán)重程度心力衰竭患者體內(nèi)生化指標(biāo)水平存在差異。本研究檢測(cè)不同心功能分級(jí)的老年心力衰竭急性發(fā)作期患者的血Na、BNP、UA水平發(fā)現(xiàn),隨著患者心功能分級(jí)增加,血BNP、UA水平升高,血Na水平降低;且Pearson相關(guān)分析結(jié)果顯示,血Na水平與心功能分級(jí)呈負(fù)相關(guān),血BNP和UA水平與心功能分級(jí)呈正相關(guān)。提示血Na、BNP及UA水平與老年心力衰竭急性發(fā)作期患者的病情進(jìn)展有關(guān),與Kaniahi等[9]研究結(jié)果一致。究其原因,一方面可能是心力衰竭急性發(fā)作期患者血液循環(huán)量下降,同時(shí)機(jī)體鹽攝入量減少,血Na水平降低導(dǎo)致腎素釋放、腎素-血管緊張素(RAS)系統(tǒng)被激活,Na+重吸收增強(qiáng)而引起體液潴留。另一方面,心力衰竭急性發(fā)作期患者心室充盈壓升高,神經(jīng)激素過(guò)度激活,機(jī)體清除BNP的頻次提高、受體下調(diào)結(jié)合力減弱,使排鈉利尿等作用變?nèi)?,從而?dǎo)致水鈉潴留加重[14-15]。因此,建議對(duì)老年心力衰竭急性發(fā)作期患者血Na、BNP、UA水平進(jìn)行檢測(cè),以評(píng)估心力衰竭患者心功能損傷程度。目前,臨床上有關(guān)血Na、BNP、UA水平與心力衰竭發(fā)病及病情關(guān)系的研究較少,因此本研究具有一定的創(chuàng)新性。
綜上所述,老年心力衰竭急性發(fā)作期患者血BNP和UA水平升高、血Na水平降低,且血Na、BNP、UA水平與患者心功能分級(jí)具有相關(guān)性,因此血Na、BNP、UA水平可以用于評(píng)估老年心力衰竭急性發(fā)作期患者心功能的損傷程度。

ObjectiveTo investigate the changes of serum levels of sodium,BNP and uric acid in aged patients with acute exacerbation of heart failure and their correlations with cardiac function.MethodsFrom 2011 to 2013 in the General Hospital of Yongrong Mining Limited Company of Chongqing,60 aged patients with acute exacerbation of heart failure were selected as A group,60 aged patients with stable heart failure were selected as B group,70 aged healthy cases were selected as C group.Serum levels of sodium,BNP and uric acid were detected and compared,and their correlations with cardiac function were analyzed.ResultsSerum levels of BNP and uric acid of A group were higher than those of B group,while serum sodium level was lower than that of B group(P<0.05);serum levels of BNP and uric acid of B group were higher than those of C group,while serum sodium level was lower than that of C group(P<0.05).In A group,serum levels of BNP and uric acid of patients withⅣ-stage cardiac function were higher than patients withⅢ-andⅡ-stage cardiac function,and those of patients withⅢ-stage cardiac function were higher than patients withⅡ-stage cardiac function(P<0.05);serum sodium level of patients withⅣ-stage cardiac function was lower than patients withⅢ-andⅡ-stage cardiac function,and that of patients withⅢ-stage cardiac function was lower than patients withⅡ-stage cardiac function(P<0.05).Pearson correlation analysis showed that,serum sodium level was negatively correlated with cardiac function of aged patients with acute exacerbation of heart failure(r=-0.602,P=0.006),while serum BNP level(r=0.551,P=0.012)and serum uric acid level(r=0.551,P =0.012)was positively correlated with cardiac function,respectively.ConclusionSerum levels of BNP and uric acid of aged patients with acute exacerbation of heart failure are high,while the serum sodium level is low,but all of them are correlated with the cardiac function.
