壓力控制容量保證通氣與容量控制通氣在俯臥位脊柱手術(shù)中的效果比較

摘要目的:比較俯臥位脊柱手術(shù)患者采用壓力控制容量保證通氣(pressure-controlled ventilation-volume guarantee,PCV-VG)模式與容量控制通氣(volume-controlled ventilation,VCV) 模式的效果。方法: 選擇全身麻醉下行擇期俯臥位脊柱手術(shù)的患者60例,美國麻醉醫(yī)師協(xié)會(American Society of Anesthesiologists,ASA)分級Ⅰ~Ⅱ級,體質(zhì)量43~80 kg。采用隨機數(shù)字表法,將60例患者分為VCV組和PCV-VG組,各30例。VCV組設置通氣模式為VCV,潮氣量為8 mL/kg,吸呼比為1∶2,呼吸頻率為12次/min,壓力限制設為30 cmH2O;PCV-VG組設置通氣模式為PCV-VG,其他設置同VCV組。于氣管插管后(T1)、改俯臥位(T2)、俯臥位30 min(T3)、手術(shù)結(jié)束(T4)及改平臥位(T5) 5個時間點監(jiān)測并記錄患者的心率、平均動脈壓、潮氣量、分鐘通氣量、平均氣道壓、氣道峰壓、呼吸末二氧化碳分壓,計算動態(tài)肺順應性。結(jié)果:與VCV組比較,PCV-VG組T2、T3、T4時間點平均氣道壓和氣道峰壓降低,動態(tài)肺順應性升高(P<0.05)。結(jié)論:與VCV相比,PCV-VG模式能更好地保證俯臥位脊柱手術(shù)患者的機械通氣效果,降低氣壓傷的風險,對呼吸功能的影響較小。

俯臥位能為脊柱手術(shù)提供更好的視野,方便術(shù)者的操作,是脊柱手術(shù)的一種常用體位[1]。相對于仰臥位,麻醉中俯臥位患者的胸腹部活動幅度受到限制,肺部還會受到脊柱及周圍組織的壓迫,從而影響患者的呼吸功能。有研究[2]表明,在全身麻醉機械通氣時,俯臥位能對呼吸力學產(chǎn)生較大的影響,包括呼吸道壓力上升、肺順應性下降。高氣道壓、肺順應性的變化正是導致呼吸機相關(guān)性肺損傷的相關(guān)因素。因此,全身麻醉中俯臥位下的呼吸管理是一項值得探討的課題。本研究擬在全身麻醉下行擇期俯臥位脊柱手術(shù)的患者中比較采用壓力控制容量保證通氣(pressure-controlled ventilation-volume gua-rantee,PCV-VG)模式與容量控制通氣(volume-controlled ventilation,VCV)的效果,觀察這兩種不同的呼吸模式下呼吸力學的變化,為臨床中行俯臥位手術(shù)患者的呼吸管理提供參考。
1資料與方法
1.1一般資料選擇2014年1月—2014年12月于我院在全身麻醉下行擇期俯臥位脊柱手術(shù)的患者60例,其中男性38例,女性22例;美國麻醉醫(yī)師協(xié)會(American Society of Anesthesiologists,ASA)分級Ⅰ~Ⅱ級;年齡36~62歲;體質(zhì)量43~80 kg。術(shù)前完善相關(guān)檢查,患者均無心血管疾病及慢性呼吸系統(tǒng)疾病,無頭頸部活動障礙,既往無特殊病史。采用隨機數(shù)字表法,將60例患者分為VCV組和PCV-VG組,各30例。2組患者一般資料差異無統(tǒng)計學意義(P>0.05),見表1。

1.2方法術(shù)前禁飲、禁食,術(shù)前30 min肌內(nèi)注射苯巴比妥0.1 g、阿托品0.5 mg。入室后常規(guī)開放靜脈,監(jiān)測患者的心電圖、血壓、心率、脈搏氧飽和度。有創(chuàng)血壓監(jiān)測方法:采用20 G套管針在局麻下行左側(cè)橈動脈穿刺置管,連接動脈測壓傳感器測壓。應用腦電監(jiān)測儀監(jiān)測腦電雙頻指數(shù)(bispectral index,BIS)。靜脈注射咪達唑侖0.1 mg/kg、舒芬太尼0.6 μg/kg、依托咪酯0.4 mg/kg、順式阿曲庫銨0.2 mg/kg進行麻醉誘導。氣管插管后,連接麻醉機(美國GE公司Datex-Ohmeda Advance)行機械通氣。VCV組設置通氣模式為VCV,潮氣量為8 mL/kg,吸呼比為1∶2,呼吸頻率為12次/min,壓力限制設為30 cmH2O;PCV-VG組設置通氣模式為PCV-VG,其他設置同VCV組。術(shù)中應用丙泊酚4~10 mg/(kg·h)、瑞芬太尼6~10 μg/(kg·h)和阿曲庫銨5 μg/(kg·min)進行麻醉維持。術(shù)中維持BIS值為40~60。術(shù)中根據(jù)患者基礎生命體征給予去氧腎上腺素及烏拉地爾以維持血流動力學穩(wěn)定,使血壓波動在基礎值±30%。術(shù)畢改平臥位并拔管。
于完成氣管插管后(T1)、改俯臥位(T2)、俯臥位30 min(T3)、手術(shù)結(jié)束(T4)以及改平臥位(T5)5個時間點監(jiān)測并記錄患者的心率、潮氣量、分鐘通氣量、平均氣道壓、氣道峰壓、呼吸末二氧化碳分壓,并計算動態(tài)肺順應性。
2結(jié)果
2組患者在各時間點的血流動力學指標(即心率、平均動脈壓)差異均無統(tǒng)計學意義(P>0.05),見表2。

PCV-VG組T2、T3、T4時間點氣道峰壓、平均氣道壓均明顯小于VCV組(P<0.05),動態(tài)肺順應性明顯大于VCV組(P<0.05);PCV-VG組T2、T3、T4時間點潮氣量、分鐘通氣量、呼吸末二氧化碳分壓與VCV組比較差異均無統(tǒng)計學意義(P>0.05),而2組T1、T5時間點呼吸力學指標差異均無統(tǒng)計學意義(P>0.05)。見表3。

3討論
俯臥位時患者的胸腹部活動受限,可能使肺活量和功能殘氣量降低,嚴重時甚至導致二氧化碳潴留和低氧血癥[3];同時還可能使下腔靜脈受壓,影響靜脈回流,導致血流動力學的波動。這些特點使全身麻醉下行俯臥位手術(shù)的患者的呼吸管理不同于其他體位,對機械通氣模式提出了更高的要求。
PCV-VG模式結(jié)合了VCV與壓力控制通氣(pressure-controlled ventilation,PCV)兩者的優(yōu)點,直接設置患者的潮氣量,但在送氣的氣流模式中采用了PCV模式的遞減氣流,且在通氣過程中連續(xù)測定肺順應性和氣道阻力,能根據(jù)力學變化自動調(diào)整送氣流速和氣道壓力水平,從而保證了預設的潮氣量[4]。PCV-VG模式在吸氣相是遞減氣流,在吸氣初氣道壓力即達到最大值并且維持到吸氣相結(jié)束,更利于氧的彌散。PCV-VG模式的低氣道壓更利于防止氣壓傷的發(fā)生[5]。
本研究比較了PCV-VG模式與VCV模式在俯臥位機械通氣時的效果,結(jié)果顯示,PCV-VG組T2、T3、T4時間點氣道峰壓、平均氣道壓較VCV組明顯減小,動態(tài)肺順應性較VCV組明顯增大。PCV-VG模式通過連續(xù)測定肺順應性及氣道阻力,動態(tài)調(diào)整送氣流速及氣道壓力水平,使肺內(nèi)氣體分布均勻,有效地擴張小氣道,降低了氣道阻力,改善了肺順應性。因此,在PVC-VG模式下,俯臥位患者的氣道峰壓及氣道平均壓更低、肺順應性增加,保證了患者的通氣,減少了氣壓傷的發(fā)生。
綜上所述,在脊柱手術(shù)俯臥位時采用PCV-VG模式,能夠以較低的氣道壓更有效地保證通氣量,降低氣壓傷的風險,能有效地避免肺損傷的發(fā)生,優(yōu)于VCV模式的機械通氣模式。

Effect Comparison between Pressure-Controlled Ventilation-Volume Guarantee and Volume-Controlled Ventilation during Spinal Surgery in Prone Position

AbstractObjective:To compare the effect of pressure-controlled ventilation-volume guarantee(PCV-VG) with that of volume-controlled ventilation(VCV) in patients undergoing spinal surgery in prone position. Methods:A total of 60 patients undergoing selective spinal surgery in prone position,who were classified as GradeⅠto GradeⅡby American Society of Anesthesiologists(ASA) criteria and weighed 43-80 kg, were randomly divided into VCV group and PCV-VG group with 30 cases in each. In VCV group, the ventilation mode was VCV with tidal volume of 8 mL/kg and Inspiration/Expiration ratio of 1∶2,respiratory rate of 12 breaths/min and pressure limit of 30 cmH2O.In PCV-VG group, the ventilation mode was set as PCV-VG and the other settings were same as those in VCV group.Heart rate, mean artery pressure, tidal volume, minute volume, mean airway pressure, airway peak pressure, end-tidal pressure of carbon dioxide were recorded immediately after tracheal intubation(T1),immediately before the patients were turned to prone position(T2),30 min after the patients were turned to prone position(T3),immediately after the end of surgery(T4) and immediately after the patients were turned to supine position(T5), and dynamic lung compliance was calculated. Results: Compared with those in VCV group, mean airway pressure and airway peak pressure in PCV-VG group significantly decreased at T2,T3 and T4, while dynamic lung compliance increased(P<0.05). Conclusions: Compared with VCV,PCV-VG can guarantee the effect of mechanical ventilation,reduce the risk of airway pressure injury,and impact less on respiratory function, in patients undergoing spinal surgery in prone position.
