血清超氧化物歧化酶與同型半胱氨酸水平與心血管疾病的相關(guān)性研究

目的探討血清超氧化物歧化酶(SOD)和同型半胱氨酸(HCY)水平與心血管疾病的相關(guān)性。方法選取我院100例心血管病患者,按照疾病類(lèi)型分為急性冠狀動(dòng)脈綜合征(ACS)組、不穩(wěn)定型心絞痛(UAP)組、動(dòng)脈粥樣硬化(AS)組,另選取同期體檢健康者50例作為健康對(duì)照組,測(cè)定所有研究對(duì)象的血清SOD和HCY水平,并進(jìn)行相關(guān)性分析。結(jié)果3種心血管疾病患者的血清SOD水平低于健康對(duì)照組,HCY水平高于健康對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),與治療前比較,ACS組與AS組患者治療后血清SOD水平均升高,HCY水平均下降,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),且ACS組及AS組患者血清SOD與HCY水平均呈負(fù)相關(guān)(相關(guān)系數(shù)r 分別為0.901、0.924,P<0.05)。結(jié)論血清SOD和HCY水平檢測(cè)對(duì)ASC與AS的治療和預(yù)后有重要的臨床指導(dǎo)價(jià)值。

研究報(bào)道,高血清同型半胱氨酸(HCY)是心血管疾病的重要危險(xiǎn)因素[1],此外超氧化物歧化酶(SOD)也可以作為心血管疾病的血清學(xué)標(biāo)志物。本研究主要探討血清超氧化物歧化酶(SOD)和同型半胱氨酸(HCY)水平與心血管疾病的相關(guān)性。
1.1 臨床資料
選擇我院2010年1月~2011年12月收治的100例心血管病患者,男58例,女42例,平均年齡(51.5±5.2)歲,按照疾病類(lèi)型分為急性冠狀動(dòng)脈綜合征(ACS)組(n=32)、不穩(wěn)定型心絞痛(UAP)組(n=33)、動(dòng)脈粥樣硬化(AS)組(n=35),另選取同期體檢健康者50例作為健康對(duì)照組。
1.2 儀器、試劑和方法
采用美國(guó)Beckman Coulter 公司生產(chǎn)的AU5821全自動(dòng)生化分析儀,SOD 測(cè)定采用比色法,HCY 測(cè)定采用循環(huán)酶法。
1.3 統(tǒng)計(jì)學(xué)處理
2.1 各心血管疾病組治療前后SOD及HCY水平比較
治療前,急性冠狀動(dòng)脈綜合征(ACS)組SOD和HCY水平分別為(92.64±5.71)U/mL、(29.14±5.10)μmol/L,不穩(wěn)定型心絞痛(UAP)組SOD和HCY水平分別為(95.62±9.60)U/mL、(27.40±5.03)μmol/L,動(dòng)脈粥樣硬化(AS)組SOD和HCY水平分別為(81.03±6.30)U/mL、(32.76±8.75)μmol/L,治療后,急性冠狀動(dòng)脈綜合征(ACS)組SOD和HCY水平分別為(98.64±9.87)U/mL、(21.48±5.34)μmol/L,不穩(wěn)定型心絞痛(UAP)組SOD和HCY水平分別為(97.25±10.43)U/mL、(26.14±7.36)μmol/L,動(dòng)脈粥樣硬化(AS)組SOD和HCY水平分別為(102.35±16.45) U/mL、(20.45±5.64)μmol/L。治療前3種心血管疾病患者的血清SOD水平低于健康對(duì)照組,HCY水平高于健康對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。與治療前比較,ACS組與AS組患者治療后血清SOD水平均升高,HCY水平均下降,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。
2.2 SOD 和HCY水平的相關(guān)性分析
ACS組和AS組患者血清SOD與HCY水平均呈負(fù)相關(guān)(相關(guān)系數(shù)r分別為0.901 、0.924, P<0.05),UAP組患者血清HCY與SOD水平無(wú)相關(guān)性(P>0.05)。
SOD是一種金屬酶,廣泛地存在于自然界動(dòng)植物體內(nèi)[2],主要作用是催化超氧陰離子自由基發(fā)生歧化反應(yīng),在人體內(nèi),SOD是重要的抗氧化劑,能夠清除體內(nèi)的氧自由基,保護(hù)細(xì)胞[3]。HCY為含硫氨基酸,當(dāng)HCY代謝受阻時(shí),其在細(xì)胞內(nèi)堆積并進(jìn)入血液循環(huán),血液中的HCY水平會(huì)明顯升高[4]。本研究顯示,心血管疾病患者HCY水平升高,經(jīng)分析這和體內(nèi)的HCY代謝受阻有關(guān),HCY水平升高又會(huì)導(dǎo)致體內(nèi)生成大量的過(guò)氧化物和超氧化物,導(dǎo)致SOD水平降低。本研究顯示,治療后,ACS組與AS組患者血清SOD水平均升高,HCY水平均下降,這就提示SOD和HCY水平與ACS和AS的疾病進(jìn)程密切相關(guān),有助于了解該類(lèi)疾病的發(fā)病,并作出及早診斷和治療。

Correlation Between Serum SOD and HCY Levels With Cardiovascular Diseases

ObjectiveTo explore the correlation between the levels of serum homocysteine(HCY)and superoxide dismutase(SOD)with cardiovasculardiseases.Methods100 patients with cardiovascular diseases were selected and divided into the acute coronary syndrome(ACS)group ,unstable angina pectoris(UAP)group and the atherosclerosis(AS)group according to the disease types,contemporaneous 50 individuals of healthy physical examination were selected as the healthy control group ,the serum SOD and HCY levels were detected in all the research subjects and the correlation was analyzed.ResultsThe serum SOD level in the 3 cardiovascular disease groups was significant lower than that in the healthy control group,while the HCY level was higher than that in the healthy control group,the differences between them had statistical significance(P<0.05). Compared with those before treatment,the serum SOD levels after treatment in the ACS group and the AS group were significant increased and the serum HCY levels were significant decreased with statistical differences(P<0.05),moreover serum SOD and HCY levels showed the negative correlation in the ACS group and the AS group(r=0.901、0.924,P<0.05).ConclusionDetecting serum SOD and HCY levels could have an important guidance value to the treatment and prognosis of ACS and AS .
