富含血小板血漿外敷在深Ⅱ度燒傷創(chuàng)面修復(fù)中的臨床應(yīng)用

目的 研究富含血小板血漿(PRP)創(chuàng)面外敷在深Ⅱ度燒傷創(chuàng)面治療中的作用。方法收集軀干及四肢Ⅱ度燒傷患者68例,年齡1~65歲,燒傷面積5%~62%總身體表面積(TBSA),將每例患者的其中一處深Ⅱ度創(chuàng)面平均劃分為A、B 2個(gè)部分(治療組和對(duì)照組),確保其創(chuàng)面深度一致。取患者外周靜脈血制成富含血小板血漿凝膠。治療組用富含血小板血漿凝膠敷于創(chuàng)面,對(duì)照組用磺胺嘧啶銀霜治療。比較2組創(chuàng)面的愈合時(shí)間、創(chuàng)面愈合率、更換最內(nèi)層藥用紗布的次數(shù)、換藥次數(shù)、創(chuàng)面分泌物細(xì)菌培養(yǎng),同時(shí)監(jiān)測(cè)患者在研究期間實(shí)驗(yàn)室檢測(cè)指標(biāo)及不良反應(yīng)情況。結(jié)果治療組創(chuàng)面平均愈合時(shí)間較對(duì)照組縮短[(16.5±3.1)d vs(19.5±3.8)d],第14、17天治療組愈合率較對(duì)照組高(P<0.05)。治療組2周內(nèi)外敷料滲濕紗布層數(shù)[(20.6±1.7)層vs(23.3±5.9)層]、炎性反應(yīng)發(fā)生率(7.4%vs 19.1%)、平均換藥次數(shù)[(7.2±1.1)次vs(9.2±1.4)次]、內(nèi)層紗布更換次數(shù)[(2.3±0.6)次vs(5.3±1.3)次]均低于對(duì)照組。2組創(chuàng)面細(xì)菌培養(yǎng)陽(yáng)性率、實(shí)驗(yàn)室檢查指標(biāo)及不良反應(yīng)發(fā)生率等與對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論深Ⅱ度燒傷創(chuàng)面治療中應(yīng)用PRP創(chuàng)面外敷,能明顯縮短創(chuàng)面愈合時(shí)間,提高創(chuàng)面愈合率,減少換藥次數(shù),促進(jìn)創(chuàng)面愈合。采用PRP可安全可靠地治療深Ⅱ度燒傷。

富含血小板血漿(platelet-rich plasma,PRP)是經(jīng)過(guò)離心,從全血中得到的富含血小板的濃縮物,含有多種促細(xì)胞增殖、分化的生長(zhǎng)因子,如轉(zhuǎn)化生長(zhǎng)因子(transforming growth factor,TGF)、血小板源性生長(zhǎng)因子(palete derived growth factor,PDGF)、胰島素樣生長(zhǎng)因子(insulin like growth factor,IGF)等[1]。因PRP來(lái)源于自體,可促進(jìn)軟組織修復(fù),近年來(lái)已被應(yīng)用于普通外科、整形美容、心胸外科和骨科等學(xué)科。研究報(bào)道,PRP可減少術(shù)中出血、術(shù)后傷口滲出、減輕疼痛、減少術(shù)后并發(fā)癥、促進(jìn)創(chuàng)面修復(fù)及術(shù)后組織功能恢復(fù)等[2]。近年來(lái),我院燒傷外科采用PRP處理深Ⅱ度燒傷創(chuàng)面,效果良好,現(xiàn)報(bào)告如下。
1.1 一般資料 收集2011年3月—2013年9月入住我院燒傷整形科的深Ⅱ度(創(chuàng)面深度由燒傷外科2名主治醫(yī)師按燒傷“三度四分法”進(jìn)行判斷,診斷一致方可確診)燒傷患者68例,其中女28例,男40例,年齡1~65歲,平均(35.3±14.2)歲;其中火焰燒傷25例,開(kāi)水燙傷38例,電燒傷3例,化學(xué)性燒傷2例;燒傷總面積5%~62%總身體表面積(total body surface area,TBSA),平均燒傷面積(18.1±12.6)%TBSA,傷后2 h內(nèi)就診,排除其他疾病,患者燒傷部位均位于軀干及四肢。
1.2 方法
1.2.1 分組 將每例患者的一處深Ⅱ度創(chuàng)面(面積大于1%),人為劃分為面積相等2部分,分別標(biāo)記為A、B部分,確保2部分創(chuàng)面深度一致,分別設(shè)為治療組和對(duì)照組,其他創(chuàng)面作為觀察創(chuàng)面。
1.2.2 創(chuàng)面處理方法 用0.9%氯化鈉溶液+0.5%碘伏清創(chuàng),去除所有腐皮,治療組將PRP凝膠貼于創(chuàng)面,凝膠制作及應(yīng)用在30 min內(nèi)完成,每隔7 d更換1次,3次治療為1個(gè)療程,創(chuàng)面用10層無(wú)菌紗布覆蓋,每日更換外敷料,1周后更換外敷料直至愈合。對(duì)照組清創(chuàng)后,用1%磺胺嘧啶銀冷霜外敷于創(chuàng)面,10層無(wú)菌紗布覆蓋,每日換藥直至愈合。
1.2.3 PRP凝膠制備 取外周靜脈血30 mL,加枸櫞酸鈉3 mL抗凝。200×g離心10 min,棄去離心管最下面的紅細(xì)胞層至交界面下3 mm,剩下的樣品以200×g離心10 min,下層即為PRP,約5 mL,加凝血?jiǎng)? mL 10%的氯化鈣+1 000 U凝血酶),按照1 mL∶200 U制成PRP凝膠。
1.2.4 觀察指標(biāo) (1)統(tǒng)計(jì)創(chuàng)面平均愈合時(shí)間并計(jì)算愈合率。2組患者創(chuàng)面愈合時(shí)間由2名主治以上醫(yī)師進(jìn)行判斷。去除敷料后,一致認(rèn)為創(chuàng)面達(dá)到95%的上皮化為愈合判斷標(biāo)準(zhǔn),并記錄愈合時(shí)間。分別于治療后的第11、14、17天根據(jù)計(jì)算創(chuàng)面愈合率的公式([用藥前準(zhǔn)備用于實(shí)驗(yàn)的創(chuàng)面總面積-未愈創(chuàng)面面積)/用藥前準(zhǔn)備用于實(shí)驗(yàn)的創(chuàng)面總面積× 100%]計(jì)算創(chuàng)面愈合率。(2)敷料的滲液及創(chuàng)周炎性反應(yīng)情況。計(jì)數(shù)1周內(nèi)外敷料紗布滲濕層數(shù);判定是否存在創(chuàng)周炎性反應(yīng)。(3)換藥的次數(shù)和更換內(nèi)層紗布的次數(shù)。統(tǒng)計(jì)2組患者在治療過(guò)程中的換藥次數(shù)和內(nèi)層紗布更換的次數(shù)。(4)創(chuàng)面細(xì)菌培養(yǎng)的陽(yáng)性率。于第5、12天分別在A、B創(chuàng)面上取分泌物進(jìn)行細(xì)菌培養(yǎng),比較兩者細(xì)菌培養(yǎng)的陽(yáng)性率。(5)監(jiān)測(cè)2組患者在治療過(guò)程中血常規(guī)、肝腎功能的變化,并觀察患者有無(wú)其他不良反應(yīng)。
1.3 統(tǒng)計(jì)學(xué)方法 采用SPSS 13.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)分析,計(jì)量資料采用均數(shù)±標(biāo)準(zhǔn)差(±s)表示,組間比較采用兩獨(dú)立樣本t檢驗(yàn),計(jì)數(shù)資料采用例表示,組間比較采用χ2檢驗(yàn),P<0.05表示差異有統(tǒng)計(jì)學(xué)意義。
2.1 2組創(chuàng)面平均愈合時(shí)間及愈合率的比較 觀察結(jié)果顯示,2組患者在治療的第11天,其愈合率之間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義,而在治療第14和17天,治療組創(chuàng)面愈合率高于對(duì)照組,且治療組創(chuàng)面愈合時(shí)間短于對(duì)照組(P<0.05),見(jiàn)表1。

2.2 2組患者的敷料滲液情況及創(chuàng)周炎性反應(yīng)比較 治療組2周內(nèi)外敷料滲濕紗布層數(shù)為(20.6± 1.7)層,對(duì)照組(23.3±5.9)層,治療組的滲濕紗布層數(shù)明顯少于對(duì)照組(t=2.136,P<0.05)。于治療2周后觀察發(fā)現(xiàn)治療組患者有5處(7.4%)出現(xiàn)炎性反應(yīng),對(duì)照組13處(19.1%),前者炎性反應(yīng)陽(yáng)性率少于對(duì)照組(χ2=4.098,P<0.05)。
2.3 2組患者的創(chuàng)面換藥次數(shù)及內(nèi)層紗布更換次數(shù)比較 治療組患者的平均換藥次數(shù)為(7.2±1.1)次,內(nèi)層紗布更換次數(shù)為(2.3±0.6)次;對(duì)照組患者的平均換藥次數(shù)為(9.2±1.4)次,內(nèi)層紗布更換次數(shù)為(5.3±1.3)次。治療組患者的創(chuàng)面換藥次數(shù)和內(nèi)層紗布更換的次數(shù)均明顯低于對(duì)照組(t分別為2.184和3.207,P<0.05或P<0.01)。
2.4 2組患者創(chuàng)面細(xì)菌培養(yǎng)的陽(yáng)性率比較 對(duì)照組在治療第5天有2例(2.9%)細(xì)菌培養(yǎng)陽(yáng)性、第9天有1例(1.5%)細(xì)菌培養(yǎng)陽(yáng)性,而治療組在治療第5天有1例(1.5%)細(xì)菌培養(yǎng)陽(yáng)性、第9天有1例(1.5%)細(xì)菌培養(yǎng)陽(yáng)性,其余均為陰性。2組不同時(shí)間陽(yáng)性率差異無(wú)統(tǒng)計(jì)學(xué)意義(χ2分別為0.341和0.000,P>0.05)。
2.5 2組患者實(shí)驗(yàn)室檢查指標(biāo)及不良反應(yīng)發(fā)生率比較 2組患者血常規(guī)中紅細(xì)胞、白細(xì)胞、血小板計(jì)數(shù)及中性粒細(xì)胞、淋巴細(xì)胞與單核細(xì)胞在白細(xì)胞中所占比例均在正常范圍內(nèi)。肝腎功能檢查中丙氨酸轉(zhuǎn)氨酶(ALT)、天門冬氨酸轉(zhuǎn)氨酶(AST)、血尿素氮(BUN)、血肌酐(Scr)、血尿酸等肝腎功能損傷相關(guān)指標(biāo)無(wú)異常波動(dòng)。治療過(guò)程中未出現(xiàn)過(guò)敏、疼痛等不良反應(yīng),對(duì)照組有3例發(fā)熱1次,經(jīng)退熱處理后無(wú)再發(fā)熱。

3.1 PRP在創(chuàng)面修復(fù)中的作用 創(chuàng)面的修復(fù)是一個(gè)非常復(fù)雜的過(guò)程。主要依靠炎性細(xì)胞、修復(fù)細(xì)胞、胞外基質(zhì)及生長(zhǎng)因子的協(xié)同作用來(lái)重建受損的軟組織,其中血小板來(lái)源的生長(zhǎng)因子起到了重要作用[1]。研究表明,PRP應(yīng)用于傷口修復(fù)治療可以刺激血管再生、縮短傷口炎癥反應(yīng)期、增加膠原沉積、增加早期傷口的強(qiáng)度。PRP可以加速創(chuàng)面表皮化生長(zhǎng)、減少術(shù)后傷口滲出、減輕創(chuàng)傷后傷口腫脹和疼痛[3]。有多項(xiàng)研究證實(shí),PRP可促進(jìn)難治性潰瘍,包括糖尿病足潰瘍的愈合[4]。PRP可加快上皮細(xì)胞增殖,促進(jìn)傷口膠原合成及血管再生,而內(nèi)含大量的生長(zhǎng)因子可能彌補(bǔ)傷口處生長(zhǎng)因子過(guò)少,從而能較快地啟動(dòng)修復(fù)機(jī)制,為傷口修復(fù)提供較好的微環(huán)境和血供[5]。此外,使用PRP治療傷口瘢痕少,將PRP用于外科手術(shù)中可減少出血、預(yù)防傷口感染[6]。
3.2 深Ⅱ度燒傷的概況 深Ⅱ度燒傷的損傷達(dá)真皮層,燒傷的深淺不一,有些稍偏淺,損傷達(dá)真皮淺層,有些偏深,損傷達(dá)真皮深層,殘留部分皮膚附件,故深Ⅱ度燒傷的臨床變異較多。深Ⅱ度燒傷創(chuàng)面微濕,紅白相間,或有粟粒樣密集小紅點(diǎn),痛覺(jué)遲鈍。因?yàn)橛袣埩舻钠つw附件及部分真皮層,因此如未感染使損傷加深,則無(wú)需植皮可愈合[7]。
3.3 PRP治療深Ⅱ度燒傷療效的評(píng)估 本研究顯示,PRP治療組患者創(chuàng)面平均愈合時(shí)間及愈合率均優(yōu)于對(duì)照組。且文獻(xiàn)報(bào)道,PRP能減少傷口滲出和出血[8]。本研究顯示,PRP治療組患者換藥次數(shù)及更換內(nèi)層紗布次數(shù)少于對(duì)照組,而出現(xiàn)炎性反應(yīng)情況的患者比例也少于對(duì)照組。但2組患者創(chuàng)面細(xì)菌培養(yǎng)的陽(yáng)性率未見(jiàn)差異,血常規(guī)和肝腎功能以及不良反應(yīng)與對(duì)照組之間亦無(wú)差異。2組患者在治療過(guò)程中未出現(xiàn)過(guò)敏、疼痛等不良反應(yīng),但對(duì)照組有3例發(fā)熱1次,經(jīng)退熱處理后無(wú)再發(fā)熱,考慮為燒傷吸收熱。由此可見(jiàn),PRP治療深Ⅱ度燒傷患者的療效好于對(duì)照組,并且是安全可行的。隨著對(duì)PRP作用機(jī)制研究進(jìn)一步深入,PRP的性能將會(huì)被更好地開(kāi)發(fā)出來(lái)。

Clinical Application of Platelet-Rich Plasma in Treatment of Deep GradeⅡBurn Wounds

ObjectiveTo evaluate the clinical effects of platelet-rich plasma in treatment of DeepⅡburn wounds.MethodsSixty eight cases of inpatients with DeepⅡburns were selected,whose age were from 1 to 65 years and their burn areas were between 5%~62%of total body surface area(TBSA).Deep gradeⅡburn of each sample was divided into two parts.Part A was the treatment group and part B was the control group.The burn wounds in the treatment group were treated with platelet-rich plasma and the counterpart in the control group were treated with SD-Ag.Healing time,recovering rate and the frequency of dressing changes,frequency of changing the most innerlayer gauze and the rate of wound infection were also analyzed.Finally the laboratory abnormalities and adverse effect were monitored regularly.ResultsThe healing time of the treatment group(16.5±3.1)d was shorter than that of the control group(19.5±3.8)d.The recovering rate of the treatment group was higher than that of the control group on the 14thand 17thday after treatment.There were statistically significant differences between these two groups(P<0.05).After two weeks’treatment,the internal and external dressing permeability into wet gauze layers were 20.6±1.7,which were significantly decreased than that in the control group 23.3±5.9. The frequency of dressing changes was(7.2±1.1)times in treatment group versus(9.2±1.4)times in control group and the frequency of changing the inner most layer gauze was(2.3±0.6)times in the treatment group versus(5.3±1.3)times in control group.There were 5 inflammation reaction cases in the treatment group,but 13 cases in the control group.However,there was no statistic significance between the two group in the outcomes of bacterial culture,the laboratory abnormalities and the adverse effect.ConclusionPlatelet-rich plasma can remarkably shorten the healing time,improve healing rate,reduce frequency of dressing change and promote wound healing for deepⅡburn wound.PRP is a potential safety reagent in treating deepⅡburn wound.
